Utilize Client Forms to easily collect information on a client!
NOTE: Currently, only those with a Hi Rasmus user account can complete a client form.

The Functionality of Forms:
- Send questions to users to complete (Example: intake form, updated insurance information, etc).
- The information within the form can be used to update the Client's Profile automatically.

Steps to Create Forms
Note: Manager-level users have access to create forms and the ability to give other users access to creating forms.
Contact your organization's manager for access.
1. Navigate to Forms by selecting [Settings] >> {Templates] >> [Forms]
Layout Settings
If you would like to create settings custom to you and your organization, you can select the [Layout Settings] in order to create a header or footer you would like on every export or when rendering to an integration.
2. Next, select [Create a Form Template]
3. Name your template within the [Template Name] section.
Steps to Adding Dynamic Fields & Custom Fields to Forms
Dynamic Fields
1. Select the [Dynamic Fields] within your form when creating or updating your form.
2. Click on the Dynamic Field you would like in the Form. This will be displayed by the purple chip within the Form.
Custom Field
1. Click [Add custom field].
While creating or managing a custom field, you can create custom fields and prompts to easily have others enter data within the field. Make sure each section is filled out properly.
Once your data field is complete and saved, you can go in and add the custom field you just created within the [Dynamic Fields] tab.
If you have created a custom field or are using a custom field, there will be a pencil icon to the right of the field. You can edit these fields at any time.
Reminder: If you would like the information gathered within the field to automatically update once completed, make sure to select the [Synchronize input value to profile after form completion] checkbox prior to selecting save.
TIP: If you want to make sure this area is always filled out on your form, and the user cannot move forward until this area is complete, select the [Required Field] checkbox.
6. If you would like to require a signature, select the [Require Signature], and select the users you would like to require signatures from.

7. If you would like to lock the field, and do not want the information to be changed/edited once submitted, select [Lock content from manual editing] within the [Content & Permissions].
8. Click, [Save]
How to Generate a Form:
1. Navigate to the [Files] section for the client by selecting the client from the dropdown >>[Files] >> [New Form]
2. Once you select [New Form], fill out the required information. Include the template you would like to use that you have created, updating all relevant information.
NOTE: You do not have to assign the form to a particular user. You can assign the form to parents, caregivers, technicians, etc. If you leave the [assign form to] selection blank.
3. Once you are ready to make the form available, select [Add Form].
Where to Access the Form
1. Once the form has been added to the learner, it will appear above the sessions on the clientโs home page. Below is an example for reference:
2. The user will be directed to complete all fields, including any required fields.
NOTE: If the user does not complete all fields, manager-level users can end the form on behalf of another user.
Accessing the Form Once it is Complete
1. Select the client from the dropdown >> {History] you will be able to access the relevant information about the form, and when it was completed. You may also click on the form from the list, and see the form details, including the date and time of completion.
This form can also be found within the [Files] section within the learnerโs page.
If you would like to download this form, you can do so by selecting the [Download Icon] to the right of the file.
